I’ve only had one dermatologist actually look at my lesions. She is not convinced. She thinks I have been scratching. They are like flat scabbed areas but no nodules and they don’t look like the pictures

Can a biopsy lead to a misdiagnosis of prurigo nodularis?
A biopsy is actually one of the most helpful tools for accurately diagnosing PN, not misdiagnosing it. By examining skin cells under a microscope, dermatologists can identify specific characteristics that distinguish PN from other conditions that look similar.
PN skin typically shows:
* Higher levels of keratin (hyperkeratinization)
* Increased inflammatory cells like macrophages, mast cells, and neutrophils
* Fibrosis (thickened or scarred skin)
That said, PN can look very similar to other skin conditions visually, which is why a biopsy helps confirm the diagnosis. Conditions that mimic PN include atopic dermatitis, psoriasis, lichen simplex chronicus, and pemphigoid nodularis.
If your dermatologist doesn't think it's PN based on examining your lesions, they may be seeing characteristics that suggest a different condition. A biopsy could help clarify what's actually going on with your skin.
It's worth having a conversation with your dermatologist about what they do think it might be and whether a biopsy would be helpful in your case. Getting the right diagnosis is crucial for finding a treatment plan that actually works.
