Camilla, I have a standard soap box. If the biologic alone is not doing enough, add a second and possibly third layer of internal protection. Many of us take N-acetylcysteine (NAC) a derm-recommended anti itch OTC. Generally dose is 1200/mg day. Others like me, needed a third layer of protection. I take Allegra (generic), others take Zyrtec.
Those three layers have done wonders for me. Very little itching and a lot of healing.

Hi Camilla! By any chance, have you tried prescription Derma Smooth? I have found that it has helped me tremendously.

It's great to hear that Nemluvio has been making such a difference after 6 months — less itch and nodules drying up is a real win! Scalp itching can be one of the more stubborn symptoms to shake, so you're definitely not alone in that.

A MyPrurigoTeam member once shared: "I was just diagnosed yesterday, and my scalp is covered. The pain and itching are unbearable." — so the fact that yours has improved overall is meaningful progress. For that lingering scalp itch, here are some things that may help alongside your treatment:

- Cool it down — Apply a cold, wet cloth or ice pack for 5–10 minutes when itching flares
- Lukewarm showers — Hot water can trigger itching, so keep it warm, not hot, and limit to 10 minutes
- Fragrance-free products — Switch to gentle, fragrance-free shampoos with a pH between 4.5 and 6.0
- Lightweight head covering — A breathable covering can help prevent scratching while sleeping or during flares
- Avoid heat and sweating — These are known triggers that can worsen scalp itching specifically

It's worth mentioning the ongoing scalp itch to your dermatologist too — they may be able to add a targeted topical treatment to complement your Nemluvio.
