First — the mention of your brother contemplating not living is something that needs immediate attention. Please reach out to the 988 Suicide & Crisis Lifeline by calling or texting 988. This level of suffering is real, and he deserves urgent support right now. The relentless itch and sleep deprivation from prurigo nodularis can be absolutely devastating — you're a wonderful advocate for being in his corner.

Regarding additional medications that a doctor might consider alongside Nemluvio, there are several options worth discussing:

- Gabapentin — Yes, this is a real option. Because PN may involve hypersensitive nerve fibers, doctors sometimes prescribe nerve-targeting medications like gabapentin to help reduce the itch signals between the skin and brain
- Antihistamines — These can help with itch and may also support sleep
- Oral immunosuppressants — For severe cases, doctors may consider medications like cyclosporine, methotrexate, or azathioprine
- Antidepressants — These can help manage both the itch and the emotional toll of PN

It's really important that his dermatologist knows how severe things have become — both the uncontrolled itch *and* his mental state. A dermatologist and mental health professional working together can make a real difference.

I've also found that using a factor 50 sunscreen can also calm down the itch. My dermatologist told me to always use it before going outside but it also works when I'm inside. Don't know if this is of any help to anyone but I just thought I'd mention it

I've been on gabapentin for 22 years and have recently started on methotrexate and folic acid too. The two together seem as if they might work if they can get the dose right. I also use biofreeze, lanacane, probotic 0.1% and lots of moisturiser. I've been up and awake all night with it on my arm so I really feel your pain (no punn intended)! I hope you find something that helps it's so miserable feeling the way your brother feels now, it sounds as though he has someone who really cares in you and that means so much.

Along with Nemluvio, I was put on Roflumilast and that has helped along with low-dose Naltrexone (4.5 mg).
