he's been struggling with this for more than two years, dupixent did not work and he's just recently trying the UVB light for three weeks and I don't think that's doing much either help

Use clobetasol ointment (Rx) and cover with an occlusive dressing (available on Amazon).
Hi Myla! Similar to Marc, I use NAC in conjunction with an allergy medication (Zyrtec). It has greatly reduced the itching.
Hi Marc, Thank you! Others have mentioned this but seems I need to try it out. I have not taken these together, I use to take Zyrtec for seasonal allergies and maybe that is why I don't recall so much itch.

Myia, are you taking anything besides the Nemluvio? Many of us have found that the biologic is not enough and supplement it with N-acetylcysteine (NAC) which is a derm recomended OTC itch relief; the general dose seems to be 1200 mg/day. Also, some, like me, also take allegra or zyrtec. For me those three layers of internal protection have really turned the trick on my PN.
I use capsaicin at night, I put a layer on and then put sleeves on so I don't scratch. I found if I do this consistently my flare up calms done and starts improving. I am also on nemluvio but I have seen the most help from the cream. I also keep a tube of Aveeno oatmeal lotion next to my bed because there are times I wake up from that itchy feeling and will put some lotion on. This combination has been a big win for me.
