I get the nodules and scabs like normal PN , but the last 2 weeks, I am literally breaking out in true welts all over my back , legs and stomach . I am going crazy with the itching and nothing is helping .

It sounds like you might be having histamine intolerance. It can cause hives, welts, and a rash like reactions. Benedryl will help temporarily, but switching to a no histamine diet is the best idea.
